NEVER use your hands to apply make-up, the skin on our skin harbors bacteria that you do not want anywhere near your face. Either use brushes or a fresh sponge to apply your foundation and all make-up. Personally I like my E.L.F. foundation brush, but there are many companies that make wonderful brushes.Are brushes better for mixing and applying foundation/concealer or just hands?
Brushes! Makes it smoother and not patchy.

Just use fingers. If your foundation is good, it should make no difference whatsoever-
Brushes are better as long as you use them correctly and have a good quality brush.
